A sleepy pilot who approached the runway at the wrong angle and ignored warning signs was to blame for a passenger plane crash in southern India in May that claimed 158 lives, reports said Wednesday.

A Court of Inquiry probe concluded the Air India pilot Zlatko Glusica, from Serbia, was asleep for much of the three-hour flight and was "disorientated" when the plane started to descend, the Hindustan Times reported.

The low-cost Air India Express plane flying from Dubai to the city of Mangalore overshot the runway, plunged into a gorge and burst into flames. Eight people survived the inferno.

The official crash report, which has not been released publicly, was submitted to the civil aviation ministry on Tuesday.

Two Airnorth pilots were killed on Monday, when carrying out engine failure drills in their Embraer 120 Brasilia aircraft.  There was no one else on board the 30-seat turboprop.  It is understood that the pilots were carrying out practice engine failures during the take-off roll at the time of the incident.

A female Qantas flight attendant has fallen from an aircraft in Australia before departure of the flight at Yulara airport.  The crew member is understood to have been attempting to close the door when the incident occurred.

It is not clear what aircraft type was involved but the local Police Officer stated that the Flight Attendant fell approximately 3 metres.  She suffered minor injuries after the fall despite falling head first.

Captain Chesley 'Sully' Sullenberger retired from his job as a US Airways captain on Wednesday with a final flight from Fort Lauderdale to Charlotte.  The First Officer on board was Jeff Skiles, the same FO who famously helped Sullenberger land a stricken A320 on the Hudson river in January 2009.
